Do I need to complete a Medical/Psychiatric History Form before my first appointment?

Yes, Dr. Earthman requires that you complete a Medical/Psychiatric History Form prior to your Initial New Patient appointment. If you are unable to complete the form online, please arrive at least 30 minutes before your scheduled appointment time to fill it out. Please note, Dr. Earthman will not be able to see you without a completed form, and you may be subject to a cancellation/rescheduling fee if you are not prepared.
